Immunization Information 2020

Dear Parents,
Lincoln County Public Health is partnering with LCSD#2, to provide a free vaccination clinic.
We will be offering the Tdap vaccine (Tetanus, Diphtheria, Pertussis), which is required for entry into the 7th grade. We will also be offering the MMR (Measles, Mumps, Rubella), HBV (Hepatitis B Vaccine), Varicella (chicken pox) and IPV (Polio) vaccine, for the students who need them.
This clinic is a public service provided by LCSD#2 and LCPH with vaccines provided by the Wyoming Department of Health’s Vaccines for Children Program. There is no charge for this service.
For all students who are not compliant with the required immunizations or who do not have up to date records at the school, a letter has been sent to you regarding which vaccines are missing. If you have questions regarding this letter, or the vaccines your child needs, please call nurse Erin Ragain at SVMS or you can bring a copy of the vaccine administration record to the school.
Vaccination is the safest and most effective way to protect your student from many serious and potentially fatal diseases. To protect as many students as possible, the immunization clinic will be held at Star Valley Middle School on Thursday September 24th, starting at 9 am. 
If you would like your child to receive their required vaccines at this clinic, please:
*Review the Vaccine Information Statement (VIS) and the Notice of Privacy Practices, and
*Print and fill out all three pages of the attached consent form (signed) and mail or hand deliver to SVMS by 9/16/20.
*We will only vaccinate children whose consent forms we have received by 9/16/20.
